29 CFR 1910.147(d)(3): All energy isolating devices that were needed to control the energy to the machine or equipment was not physically located and operated in such a manner as to isolate the machine or equipment from the energy source: a) 1617 Matassino Road, New Castle, Delaware - On or about February 11, 2019, the employer did not ensure that all energy sources were isolated when an Ariens Snow blower was being maintained in the field resulting in the unexpected startup of the machine resulting in an employee suffering an injury.
