On or about January 15, 2019, the employer did not ensure the electrical energy isolating device (cord and plug) was operated in a manner to isolate the electrical power of a STA Packaging Taping (case sealer) machine, Model No. STA-37, Serial No. At021932, while a maintenance leadman performed service and maintenance (unjamming) operations.
