29 CFR 1926.964(b)(5): Reel-handling equipment, including pulling and tensioning devices, was not in safe operating condition and leveled and aligned: (a) On or about 01/24/2019 - 1085 Tingle Tangle Road, Vance, Alabama, employees installing fiber optic cable were exposed to struck-by hazards due to a modified transmission block (sheave assembly) not being in a safe operating condition.
