29 CFR 1910.28(b)(3)(ii): The employer did not ensure each employee is protected from tripping into or stepping into or through any hole that is less than 4 feet (1.2 m) above a lower level by covers or guardrail systems. On or about January 24, 2019: Employees working in the final finish area are exposed to fall hazards where they work on an elevated platform that is 36 inches high and approximately 2-feet away from the manufactured house they must go in and out of to add appliances and do the final cleaning. The employer does not ensure that the metal plates that the employees walk over has safety rails.
