29 CFR 1910.178(a)(4): Modifications and additions which affect capacity and safe operation of powered industrial truck were performed by the employer without the manufacturer's prior written approval: (a) In the warehouse area, between December 24, 2019 and January 24, 2019 and at times prior to, the employer allowed employees to utilize a Caterpillar powered industrial truck Model SDS20 with the forks on upside down to remove industrial bags from a cargo trailer without the manufacturer's prior written approval, exposing employees to struck-by hazards.
