29 CFR 1910.95(c)(1): The employer did not administer a continuing, effective hearing conservation program as described in 29 CFR 1910.9(c) through (o) whenever employee noise exposures equal or exceed an 8-hour time-weighted average sound level of 85 decibels measured on the A scale, or equivalently a dose of fifty percent: This violation occurred on March 21, 2019 when the employer did not develop and administer a hearing conservation program as described in 29 CFR 1910.95(c) through (o) for employees exposed to noise that exceeded an 8-hour time-weighted average (TWA) of 85 decibels: (a) An extruder operator working on line #2 was exposed to continuous noise levels at 58.3% of the action level. The equivalent decibel (dBA) level of 58.3% is approximately 86.1 dBA. Sampling was performed for 630minutes during one shift. The action level was adjusted from 85.0 dBA to 83.0 dBA to account for the extended shift of more than 8 hours;
