29 CFR 1910.147(d)(4)(i): Lockout or tagout devices were not affixed to each energy isolating device by authorized employees: (a) Building 4, RCL Lines - When machine operators cleaned the polish rolls at the extruder, in preparation for starting up a line, the operator locked out and tagged out the air valve for the traverse but did not lockout/tagout the other energy sources, including electricity to the polish rolls, exposing employees to machine hazards, including, getting caught in the polish rolls, on or about December 26, 2018. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and state employees and their representatives were informed of this abatement.
