29 CFR 1926.501(b)(4)(ii): Each employee on a walking/working surface was not protected from tripping in or stepping into or through holes (including skylights) by covers. On or about December 28, 2018, the employer did not ensure the 4-inch deep hole on either side of the lube pit had a cover affixed over it prior to allowing employee(s) prep the floor surface for epoxy flooring installation. This exposed employee(s) to tripping hazards.
