29 CFR 1910.147(f)(1)(ii): When lockout or tagout devices temporarily were removed from the energy isolating device and the machine or equipment was energized to test or position the machine, the employees were not removed from the machine or equipment area in accordance to the paragraph (e)(2): a) On or about 01/02/19 in the Folding area; after the repairing and restarting Gowen folder #5, a maintenance employee was exposed to caught in hazards when he was checking the functions of the machine at the discharge end and was too close to the rollers and belts catching a sleeve on the moving parts. ABATEMENT DOCUMENTATION REQUIRED
