29 CFR 1926.100(a): Employees were not protected by protective helmets while working in areas where there was a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns: a) Jobsite - On or about December 7, 2018, employees working inside an excavation that was up to 9 feet deep were not safeguarded from head injuries such as but not limited to soil from the spoil pile, thus exposing employees to struck-by hazards.
