29 CFR 1910.95(g)(1): The employer did not establish and maintain an audiometric testing program as provided by 29 CFR 1910.95(g) by making audiometric testing available to all employees whose exposures equal or exceed an 8-hour time-weighted average of 85 decibels: a) Classic Window Coverings, LLC - An employee blowing off dust from panels was exposed to noise at a time weighted average (TWA) of 95.8 dBA, which exceeded the extended 10 hours action level of 83.4 dBA. The exposure was derived from a sample taken over 558 minutes. An audiometric testing program was not established, on or about, 12/17/18. b) Classic Window Coverings, LLC - An employee operating the route hole machine was exposed to noise at a time weighted average (TWA) of 90.6 dBA, which exceeded the extended 10 hours action level of 83.4 dBA. The exposure was derived from a sample taken over 553 minutes. An audiometric testing program was not established, on or about, 12/17/18. c) Classic Window Coverings, LLC - An employee operating the louver LM machine was exposed to noise at a time weighted average (TWA) of 86.5 dBA, which exceeded the extended 10 hours action level of 83.4 dBA. The exposure was derived from a sample taken over 544 minutes. An audiometric testing program was not established, on or about, 12/17/18. d) Classic Window Coverings, LLC - An employee supporting the louver machine was exposed to noise at a time weighted average (TWA) of 90.5 dBA, which exceeded the extended 10 hours action level of 83.4 dBA. The exposure was derived from a sample taken over 542 minutes. An audiometric testing program was not established, on or about, 12/17/18.
