29 CFR 1910.178(a)(5): The powered industrial truck was equipped with front-end attachments other than factory installed attachments; however, the employer did not request that the truck be marked to identify the attachments and show the approximate weight of the truck and attachment combination at maximum elevation with load laterally centered: (a) Warehouse - The Toyota sit-down rider forklift, model 8FGCU25, serial number 66551, did not have a nameplate and markings for its Rightline single-double attachment but rather had a nameplate and markings for a side-shifter attachment, exposing employees to the hazard of overloading the forklift, on or about November 28, 2018. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and state employees and their representatives were informed of this abatement.
