29 CFR 1904.39(a)(1): The employer did not report within 8-hours the death of an employee resulting from a work-related incident. Located at 289 Elm Street, Marlborough, MA On or about November 21, 2018, the following workplace fatality was not reported to OSHA within 8 hours of knowledge of the fatality. On or about November 7, 2018, an employee received a contusion on the head from bumping into the rear roll-up door of a box truck and died eight days later on November 15, 2018, due to blunt head trauma and a left epidural hematoma.
