On October 30th 2018 in the B&H Freight Line Inc. warehouse located at 1700 Anaconda Road, Harrisonville Missouri 64701, an employee was exposed to electrocution hazards. The tag out device attachment in the electrical breaker box was not substantial enough to prevent inadvertent or accidental removal.
