29 CFR 1910.147(c)(7)(i)(A): Authorized employee(s) did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation: On or about October 22, 2018, the employer failed to ensure the employees were trained and they had the knowledge and skills for the safe application, usage and removal of energy control devices when performing maintenance, unjamming or die changes on equipment. Employees were exposed to the hazard of being caught-in the machine and /or an electrical shock hazard from an unexpected start-up of the machine.
