29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s): Sheet Metal Area, Vytek Systems, Inc., 195 Industrial Road, Fitchburg, Massachusetts; On and before October 26, 2018, employees were exposed to amputation hazards from unguarded points of operation on sheet metal machines. a. Amanda Press Brake - The point of operation on the bottom activated press brake was not protected to prevent employee access during operation. b. Niagra Metal Shear - The point of operation had stop pins, but no guard to prevent employee access to the shear blades during operation.
