29 CFR 1926.451(g)(1)(vii): Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet (3.1 m) above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section: The employer is failing to protect employees from fall hazards. This was most recently documented on or about October 17, 2018 and October 29, 2018 at the residential construction jobsite located at 522 Iowa Ave., in Holton, Kansas. An employee was fatally injured when the all-terrain fork truck overturned. Employees engaged in re-roofing activities were lifted in/working from a work platform attachment on the fork truck and were exposed to fall hazards of approximately 27 feet. Employees were not provided with a fall protection system while working at height in the work platform.
