29 CFR 1910.28(b)(3)(v)(A): The employer did not ensure each employee was protected from falling through a hatchway and chute-floor hole by a hinged floor-hole cover that meets the criteria in � 1910.29 and a fixed guardrail system that leaves only one exposed side. When the hole was not in use, the employer did not ensure the cover was closed or a removable guardrail system was provided on the exposed sides. a. On or about October 5, 2018, in the production area, operators working on Line 7 were exposed to falls into the chutes that were opened for scrap to fall though during some production.
