29 CFR 1910.25(b)(7): Standard stairs were not used to provide access from one walking-working surface to another when operations necessitate regular and routine travel between levels, including access to operating platforms for equipment: (a) On or about 9/17/2018 - Refractory Mixer Area, employees were required to climb a portable ladder on the west side of the cement mixer approximately 2 to 4 times an hour or 16 - 32 times in an eight-hour shift from the walking-working surface of the concrete floor to the walking-working surface of the third or fourth rung of the ladder and use a tool to push the wet cement mix to the bottom of the final dispending hopper.
