29 CFR 1926.453(b)(2)(v): A body belt with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift: On or about September 07, 2018, at 65433 Bayou Rd., Plaquemine, LA, the employer failed to ensure that employees wore fall protection while working from the elevated bucket of a JLG (Model 450AJ) aerial lift. The employees were installing pipes on a sewer tank.
