29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his work environment to control or eliminate any hazards or other exposure to illness or injury. (a) Mountain Top Homes at 4273 Spruce Way in Vail CO 81657: On or before August 31, 2018, the employer did not ensure that each employee on the jobsite was trained on safety and health hazards in trenching operations. Two employees were installing water lines in trenches greater than 6 feet deep without any safety training regarding the hazards associated with this work. This condition exposed employees to cave-in and struck-by hazards.
