29 CFR 1910.212(a)(2): Guards were not affixed to the machine where possible and secured elsewhere if for any reason attachment to the machine is not possible. The guard was not such that it does not offer an accident hazard in itself. a. On or about September 13, 2018, employees were operating The Lewis Straightener Machine, and the guard on the machine was not secured, thereby exposing employees to amputation hazards.
