29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his work environment to control or eliminate any hazards or other exposure to illness or injury: The employer is failing to protect the employees from slip and fall, struck-by, and crushed-by hazards associated with plumbing and trenching activities. This was most recently documented, on August 14, 2018, at the workplace located at 1400 Terradyne Drive, Andover, Kansas. The employer did not provide training to employees to enable them to recognize, evaluate, and control of hazards when working in trenches. Additionally no person was assigned as a "Competent Person" and was not fully trained as such. Training did not include: 1) The identification of soil classifications 2) The visual and physical testing methods necessary to validate soil classifications 3) The identification and application of protective measures based upon the soil classification 4) The identification and control of jobsite specific hazards through the application of inspections 5) The identification and control of water accumulations 6) The assessment and control of hazardous atmospheres 7) The identification of utilities and the methods to control against these hazards :
