29 CFR 1910.147(f)(3)(ii)(D): Each authorized employee did not affix a personal lockout or tagout device to the group lockout device before working on the machine or equipment: a) Paper Machine No. 2 Shower Pump (West): On or about August 2, 2018, the employer did not ensure the maintenance employees affixed their personal locks to the respective energy isolating devices which exposed employees to injuries associated with electrical hazards and the unexpected startup and movement of the machinery components.
