29 CFR 1910.147(c)(5)(i): Locks, tags, chains, wedges, key blocks, adapter pins, self-locking fasteners, or other hardware were not provided by the employer for isolating, securing or blocking of machines or equipment from energy sources: The employer is failing to protect employees from amputation and struck-by hazards. This was most recently documented at 1351 S. Reca Ct, Wichita, Kansas, where employees were removing and replacing the Boom Hoist Cylinders on a Lull Highlander. The employees were exposed in that Locks, tags, chains, adapter pins, or other hardware were not utilized to block and secure the energy sources. a) On or about 07/27/18, while replacing the right Boom Hoist Cylinder on the Lull, Model 1044C-54. b) On or about 08/03/18, while replacing the left Boom Hoist Cylinder on the Lull, Model 1044C-54
