29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a) On or about July 17, 2018, the employer did not ensure the rotating bar stock on the ACME Screw Machine #7 was adequately guarded. An employee suffered an avulsion of the scalp while operating the machine. b) On or about July 17, 2018, the employer did not ensure the rotating scrap auger on the ACME Screw Machine #7 was adequately guarded. Employees are exposed to caught-in hazards while operating the machine. c) On or about July 17, 2018, the employer did not ensure that the ACME Screw Machine #7 and Davenport Screw Machine #2 were adequately guarded to prevent employees from accessing the rotating parts during operation. Employees were exposed to caught-in hazards from the moving equipment.
