29 CFR 1910.28(b)(1)(i): Each employee on a walking/working surface having an unprotected side or edge which was four feet (1.2m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems: Incoming Docking Area - On or about June 27, 2018, and at times prior to: a) At the loading dock adjacent to the emergency exit door, employees operating forklifts, packing pallet trucks, and moving boxes were exposed to a fall of approximately four feet and two inches where no guardrails were installed across the dock door opening, exposing employees to falls to the concrete below. b) At the loading dock where the dumpster was placed, employees discarding trash, packing pallet trucks, and moving boxes were exposed to a fall of approximately four feet and two inches where no guardrails were installed across the dock door opening.
