29 CFR 1926.502(d)(16)(iii): Personal fall arrest systems, when stopping a fall, were not rigged such that an employee could not free fall more than 6 feet (1.8 m) nor contact any lower level: a) Construction site, Madison Lansdale Station apartments, 2nd floor - On or about June 27, 2018, an employee using a rope as part of a personal fall arrest system did not utilize a rope grab or other means to adjust slack to prevent falling more than 6 feet or striking the next lower level.
