29 CFR 1910.28(b)(3)(i): The employer did not ensure each employee is protected from falling through any hole (including skylights) that is 4 feet (1.2 m) or more above a lower level by one or more of the following: covers, guardrail systems, travel restraint systems, or personal fall arrest systems, as required by sub-items A through D of this section. a) Army Radford Ammunition Plant Building 1626: On or about 06/11/2018, an employee sticking the #4 tank while standing on the distributor cone with a hole 36 inches wide and 66 inches deep was not protected from a potential fall hazard. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
