29 CFR 1910.134(e)(2)(ii): The medical evaluation did not obtain the information requested by the questionnaire in Sections 1 and 2, Part A of Appendix C of 29 CFR 1910.134: Outside the Maintenance Shop: On or about June 8, 2018 and at other times prior to, employees provided 3M NIOSH half face respirator exposed to airborne vapors while performing cleaning operations using a chemical solvent without having an OSHA respirator medical evaluation questionnaire (mandatory) prior to beginning the work exposing the workers to respiratory hazards.
