29 CFR 1910.147(f)(3)(ii): Group lockout or tagout devices were not used in accordance with procedures required by 29 CFR 1910.147(c)(4): a) Starch Department- On or about May 22, 2018, the employer failed to ensure that when sanitation and maintenance tasks were performed by a crew of more than one employee on the Mogul Machines group lockout procedures were utilized including, but not limited to the application of personal lockout and tagout devices and coordination of the workforce. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records. Ferrara Candy Company was previously cited for a violation of this Occupational Safety and Health standard or its equivalent standard, 29 CFR 1910.147(f)(3)(ii), which was contained in OSHA Inspection Number 979115, Citation 1, Item 4, and was affirmed as a final order on November 18, 2014, with respect to a workplace at 7301 Harrison St., in Forest Park, Illinois.
