29 CFR 1910.219(f)(3): Sprocket wheels and chains which were seven feet or less above floors or platforms were not enclosed: a. On or about May 22, 2018, employees working in the Press Department were exposed to injury while operating the Minster Press, Machine Number R-09, Model Number 40E-11, Serial Number 40E11-5013, because the sprocket wheels and chains were not guarded. b. On or about May 22, 2018, employees working in the Press Department were exposed to injury while operating the Minster Press, Machine Number R-08, Model Number 40E-9, Serial Number 9208, because the sprocket wheels and chains were not guarded. c. On or about May 22, 2018, employees working in the press Department were exposed to injury while operating the Minster Press, Machine Number R-02, Model Number 60E-4 1/2, Serial Number 60-4.5-2479, because the sprocket wheels and chains were not guarded.
