29 CFR 1910.147(c)(4)(ii): Procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, 29 CFR 1910.147(c)(4)(ii)(A), (c)(4)(ii)(B), (c)(4)(ii)(C) and (c)(4)(ii)(D): On or about 5/23/2018, at CGI Windows and Doors Inc., 3780 West 104th St, Hialeah, FL. Energy control procedures lacked specific requirements for testing to determine or verifying the effectiveness of lockout devices, tagout devices and other energy control measures for the control of hazardous energy for equipment such as, but not limited to: a. Urban, Automatic Four Head Butt Welding Machine, Type AKS1900H b. Urban, Automatic Four Head Butt Welding Machine, with PC Control AKS1400 ES, Serial No. C99276 c. 3C Roll Forming Machine, CNC 233, Serial No. 04/1889/12
