29 CFR 1910.28(b)(15): The employer did not ensure each employee on a walking-working surface 4 feet (1.2m) or more above a lower level is protected from falling: a) Vertical Rack System; On or about May 16, 2018, and at times prior, the employer exposed employees to fall hazards of approximately 6 feet from the next lower level, in that personal fall arrest systems were not provided to employees while hooking steel plates up to cranes while standing on an approximately 7" wide storage rack system. b) Steel Storage System; On or about May 16, 2018, and at times prior, the employer exposed employees to fall hazards of approximately 5 feet 2 1/2" from the next lower level, in that personal fall arrest systems were not provided to employees while storing and removing steel beams.
