29 CFR 1910.147(f)(1)(v): All systems were not de-energized and energy control measures reapplied in accordance with paragraph (d) of this section to continue the servicing and/or maintenance: (a) Sawmill; On or about April 30, 2018, Lockout was not reapplied on the electrical disconnect for a debris blower fan system during trouble shooting operations. A hand injury occurred between the drive pulleys and belts of the blower fan.
