29 CFR 1926.651(c)(2): A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that were 4 feet (1.22m) or more in depth so as to require no more than 25 feet (7.62m) of lateral travel for employees: On or about May 1, 2018, the following employees working in three (3) trenches were not provided with a ladder for egress, exposing employees to the hazards associate with no safe access: a) An employee was not utilizing a ladder for safe egress while working in the north trench at a depth of 5.5'. b) An employee was not utilizing a ladder for safe egress while working in the center trench at a depth of 5.5'. c) An employee was not utilizing a ladder for safe egress while working in the south trench at a depth of 5.5'. Abatement certification is required of this item in accordance with the requirements of 29 CFR 1903.19(c).
