29 CFR 1910.178(p)(1): Powered industrial truck(s) found to be in need of repair, defective, or in any way unsafe had not been taken out of service until restored to safe operating condition: A) Yard Area, on or about April 25, 2018, employees operated a Capacity powered industrial tractor #089 that had only one operating head light during hours of darkness, exposing employees to a struck by hazard. B) Yard Area, on or about April 27, 2018, employees operated a Kalmar Ottawa T2 powered industrial tractor #215 that had only one operating head light during hours of darkness and a non-functioning horn, exposing employees to a struck by hazard.
