29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6): A) Continental Structural Plastics, Inc., located at 2915 County Highway 96, Carey, OH 43316: On or about April 2, 2018, the employer did not ensure that all the steps outlined in 29 CFR 1910.147(d), required for the control of the hazardous energy sources, were performed prior to set-up activities. Employees conducted set-up activities such as, but not limited to, changing dies in the hydraulic press # 3 (Lawtomatic 1200 Ton Press; Manufacturers ID # 12001370). Employees were exposed to struck-by and amputation hazards while all hazardous energy sources were not controlled prior to conducting work. B) Continental Structural Plastics, Inc., located at 2915 County Highway 96, Carey, OH 43316: On or about April 24, 2018, the employer did not ensure that all the steps outlined in 29 CFR 1910.147(d), required for the control of the hazardous energy sources, were performed prior to set-up activities. Employees conducted set-up activities such as, but not limited to, changing dies in the hydraulic press # 9 (Williams & White 2000 Ton Press; Manufacturers ID # C-4269). Employees were exposed to struck-by and amputation hazards while all hazardous energy sources were not controlled prior to conducting work. C) Continental Structural Plastics, Inc., located at 2915 County Highway 96, Carey, OH 43316: On or about April 24, 2018, the employer did not ensure that all the steps outlined in 29 CFR 1910.147(d), required for the control of the hazardous energy sources, were performed prior to set-up activities. Employees conducted set-up activities such as, but not limited to, changing dies in the hydraulic press # 12(Williams & White 1200 Ton Press; Manufacturers ID # C-4468). Employees were exposed to struck-by and amputation hazards while all hazardous energy sources were not controlled prior to conducting work. As a result, the remaining applicable energy control elements, involving machine isolation [1910.147(d)(3)], lock-out/tag-out device application [1910.147(d)(4)], dissipation of residual energy [1910.147(d)(5)], and verification of isolation [1910.147(d)(6)] were not implemented to protect employees from struck-by and amputation hazards while employees conducted activities such as, but not limited to, changing dies in hydraulic presses # 3, # 9, and # 12.
