29 CFR 1910.333(b)(2)(iii)(A): A lock and tag were not placed on each disconnecting means used to deenergize circuits and equipment on which work was to be performed: (a) On or about April 23, 2018 for the employees exposed to electrical hazards where an electrical motor was removed from an electrical source during maintenance on an air compressor in the concrete plant utility room located at 3303 Rock Island Place, Bismarck, North Dakota.
