29 CFR 1910.147(d)(3): All energy isolating devices that were needed to control the energy to the machine or equipment were not physically located and operated in such a manner as to isolate the machine or equipment from the energy source(s): a) On or about 4/18/2018, Employee had his arm pinched in the #1 Sidematcher because he did not have the machine locked out while he was working on it with other employees. b) On or about 4/25/2018, Employees were being permitted to lock out control buttons for the Unscrambler when they need to enter it instead of locking out at a disconnect.
