29 CFR 1910.147(d)(2): The machine or equipment was not turned off or shut down using the procedures established for the machine or equipment. On or about 4/18/18 at the job site located in an open field one mile from the intersection of Cheatwood Road and Chandler Road, Ruston, Louisiana; the employer allowed an employee with affected lockout/tagout training to be exposed to the unguarded augurs #1-3 on Unit #26142 Blender Hopper ( 48" wide by 30" deep by 100" long) while removing clumped-up sand. The machine's power source was not disconnected at the driver side of the blender hopper.
