29 CFR 1910.147(c)(1): The employer did not establish a program consisting of energy control procedures, employee training and periodic inspections to ensure that before any employee performs any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative: In the facility at 252 Milford Street, Upton, MA 01568: On and before April 08, 2018, the employer did not establish an energy control program to protect employees from unexpected releases of energy while conducting servicing and maintenance on a Veto 60kW Stoker Boiler and Veto 6 Fuel Burner. Employees emptied ashes from the boiler approximately 2 times per day and performed servicing and maintenance inside the fuel burner bin 2-3 times per week.
