29 CFR 1910.28(b)(9)(i)(A): Each fixed ladder installed before November 19, 2018 was not equipped with a personal fall arrest system, ladder safety system, cage, or well: Rig # 7. Worksite at 1-30 Wewoka, Seminole County, OK. On or about April 09, 2018, an employee working from the rod basket used a fixed ladder to access the basket at a height of 75 feet and was not wearing a fall protection harness, exposing the employee to a fall hazard of more than 4 feet. Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation describing the actions it is taking to establish, communicate, and implement policies for the fall protection use for employees exposed to falls at a height of more than 4 feet.
