29 CFR 1910.28(b)(3)(ii): (ii) Each employee is protected from tripping into or stepping into or through any hole that is less than 4 feet (1.2 m) above a lower level by covers or guardrail systems; a. On or about April 9, 2018, and at times thereto, employees walking and working at loading dock number 3, adjacent to shredding operations, were exposed to a trip and fall hazard as a result of an approximately five inch unprotected floor hole along the front of the dock plate.
