29 CFR 1910.28(b)(3)(iv): The employer did not ensure each employee was protected from falling into a ladderway floor hole or ladderway platform hole by a guardrail system and toeboards erected on all exposed sides, except at the entrance to the hole, where a self-closing gate or an offset must be used: a) In the shredder motor room: On or about April 4, 2018, and at times prior thereto, employees were exposed to fall hazards of approx. 11 ft., in that a self-closing (swing) gate was not installed to protect workers on the ladderway platform while performing maintenance activities.
