29 CFR 1910.178(p)(1): Powered industrial truck(s) found to be in need of repair, defective, or in any way unsafe had not been taken out of service until restored to safe operating condition(s):: (a) Facility: On or about March 28 and 30, 2018, two employees were operating two Toyota propane powered industrial trucks (forklifts) and the forklifts had the following problems: *Toyota forklift (Serial #1-1283, Model #8FGCSU20) had a smooth metal brake and clutch pedal. The rubber pads were missing from the pedals. In addition, the back up beeper on this forklift was not working. *Toyota forklift (Serial #18306, Model #8FGCSU20) had a smooth metal brake pedal. The rubber pad was missing.
