29 CFR 1910.132(a): Protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered: (a) Loading and Unloading Truck Bay: On March 24, 2018 and times prior, employees were exposed to a fall hazard of 4 foot 2 to 4 foot 11 inches from the trailer deck to the ground below while loading and unloading flatbed trailers without the use of personal protective equipment such as a personal fall arrest system.
