29 CFR 1910.147(c)(6)(i): The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirement of this standard were being followed: The employer is failing to protect employees from struck-by and crushing hazards in that, periodic inspections were not performed annually with authorized employees to ensure they effectively utilize lockout/tagout procedures for protection from hazardous energy sources while servicing and maintaining equipment such as concrete trucks containing stored gravity and hydraulic pressure energy This was most recently documented to occur on 3/22/2018 during this inspection at the facility located at 431 A Street, Lincoln, NE 68501 where the last prior periodic inspection performed by the employer was in 2015.
