29 CFR 1910.28(b)(1)(i): Except as provided elsewhere in this section, the employer must ensure that each employee on a walking-working surface with an unprotected side or edge that is 4 feet (1.2 m) or more above a lower level is protected from falling by one or more of the following: (A) Guardrail systems; (B) Safety net systems; or (C) Personal fall protection systems, such as personal fall arrest, travel restraint, or positioning systems. a) On February 13, 2018 located at 4505 Kellogg Ave. Cincinnati, OH 45226 (Allcraft Marina) employee(s) performed sign installation associated operations on lower front unprotected side or edge of Poster #5567 stage/platform without utilizing conventional fall protection systems after assessing the stage and during shepherds hook transfer associated operations exposing employees to falls in excess of 17 feet to the ground below. b) On February 13, 2018 located at 4505 Kellogg Ave. Cincinnati, OH 45226 (Allcraft Marina) employee(s) performed sign installation associated operations on lower front unprotected side or edge of Poster sign #5568 stage/platform without utilizing conventional fall protection systems after assessing the stage and during shepherds hook transfer associated operations exposing employees to falls in excess of 17 feet to the ground below. c) On January 29, 2018 located at 919 W North Bend Rd. Cincinnati, Oh 45224 (McDonalds) employees performed sign installation associated operations on lower front unprotected side or edge of Poster sign #3337 stage/platform without utilizing conventional fall protection systems after assessing the stage and during shepherds hook transfer associated operations exposing employees to falls in excess of 13 feet to the ground below. d) On March 6, 2018 located across street from 4597 Kellogg Ave. Cincinnati, OH 45226 (Cincinnati Interiors, Ltd) an employee performed sign installation associated operations on lower front unprotected side or edge of Junior Poster sign #5577 stage/platform without utilizing conventional fall protection systems after assessing the stage and during shepherds hook transfer associated operations exposing employees to fall potentials in excess of 8 feet. e) On January 22, 2016 located at 3745 Beechmont Ct. Cincinnati, OH 45226 (Jacob Brothers Heating & Air Conditioning) employees performed sign installation associated operations on lower front unprotected side or edge of Poster sign #5501 without utilizing conventional fall protection during access between the fixed ladder and the horizontal lifeline located on the stage platform exposing employee(s) to fall potentials in excess of 25 feet.
