29 CFR 1910.178(p)(1): Powered industrial truck(s) found to be in need of repair, defective, or in any way unsafe had not been taken out of service until restored to safe operating condition(s). On February 27, 2018, and times prior thereto, a powered industrial truck was utilized without the use of side-view mirrors which limited the operators views, and had a leaking right-side hydraulic lift arm which could lead to a failure in maintaining the mast elevated. Employees were exposed to struck-by and falling object hazards.
